If you need to cancel your membership, please complete the form below to initiate the cancellation process. This form will serve as your official four-week written cancellation notice, as outlined in your membership agreement. Your membership will end four weeks from the date of form submission.

Please note:

If you have a scheduled renewal payment within this four-week notice period, it will be processed as scheduled and will serve as your final payment.
All payments are non-refundable.
For 12, 26, and 52-week Term Discount Plans, cancelling your membership before the completion of your specified term will incur an early termination fee. This fee will be equal to the standard non-discounted month-to-month rate for the period completed.
