1) If you would like to put your membership on hold instead of canceling, you can submit a membership hold request form. If you are sure you would like to cancel, simply fill out the form below. This will serve as your four week written notice.

2) Memberships with "length of agreement terms" will be canceled 28 days from the submission of the form below. Month to month memberships will be canceled 7 days from submission. Note that if you have a scheduled payment within that 28 day or 7 day period, it will be processed. All payments are non-refundable.

3) If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.

4) Cancellation of membership prior to expiration of any specified commitment period will be subject to an early termination fee. The early termination fee will be calculated as follows: the difference between your current rate and the month-to-month rate multiplied by the number of installments already processed on your agreement.
