We hate to see you go. All contracts are set to auto-renew and require you to complete this cancellation form 28 days prior to the contract end date if you wish to NOT renew.

Any contract that is not at full term (13 payments in an annual contract, 7 payments in a semi-annual or student contract) will have to be paid out before cancellation can be set.

The only exception is moving more than 20-miles away (which requires proof of new address).
