Membership cancellation requires 30 days notice. The 30 days notice will not begin and no action will be taken until this form is submitted. Informing staff of your desire to cancel via text, email, phone call or in person will have no effect on your membership status or billing. Any membership renewal payments scheduled within the 30 day notice will be processed as scheduled. All payments are non-refundable. You may continue to attend class during the time that you have paid for as long as you are in good standing with CFX members and staff.
